Acceptance of my entry into New Orleans Crescent City Fleur de lis Tennis Tournament is without assumption of responsibility of any kind by The Greater New Orleans Tennis Association, the Gay and Lesbian Tennis Alliance, its officers, the city of New Orleans, WOW Tennis Center or any public or private facility, host club, committee, or individuals connected with the New Orleans tournament. I agree to waive any and all rights and claims for bodily and emotional injury, damage to my reputation, or any other harm that I might suffer. I also waive any and all rights to claim from the GLTA, or any of the above organizations or individuals, damages which may arise as a result of reference or photographs of me which may appear in publications, websites or any media coverage of the tournament. I understand this tournament will be governing the conduct of the players and enforcing the USTA Code of Ethics and the Gay and Lesbian Tennis Alliance policies.
